Choosing home care for a loved one is a big decision. Here's exactly how it works at Agape, what we'll need from you, and what you'll get in return.

How it works

From first call to first visit, usually within a week.

Step 1 — A free phone call

Tell us what's going on

15 minutes with an RN. We'll ask about your loved one's needs, daily routine, current support, and any concerns. No pressure, no commitment.

Step 2 — In-home assessment

We come to you

A registered nurse meets with your family at the home. We assess care needs, look at safety, talk through payment options, and outline a starting care plan.

Step 3 — Caregiver match

Right person, day one

We choose a caregiver based on personality, skills, and schedule. You'll meet them before care starts. If it isn't the right fit, we re-match — no awkwardness.

PASSPORT Medicaid Waiver

Ohio's home- and community-based waiver for older adults who qualify for nursing-home level care but want to remain at home. We're an approved provider and handle the paperwork.

MyCare Ohio

Combined Medicare + Medicaid plans for adults dual-eligible for both. We're contracted with CareSource and Anthem for MyCare Ohio.

Questions families ask us most.

How quickly can care start?

For most families we can begin within a few days of the in-home assessment. Urgent situations (hospital discharge, sudden need) are usually handled same-week.

How do I know if my loved one qualifies for PASSPORT?

PASSPORT is income- and care-needs based. The simplest answer: call us, and we'll walk through it together in a few minutes. We can help with the application either way.

What if my loved one doesn't like their caregiver?

We re-match. We'd rather know early than have anyone uncomfortable in their own home. Continuity matters, but only when the chemistry is right.

Are caregivers background-checked and trained?

Yes. Background checks, references, and training before they ever see a client. Ongoing training in dementia care, transfers, and safety. An RN is involved in every care plan.

Can you help with overnight or 24/7 care?

Yes. We staff overnight shifts, weekends, holidays, and live-in arrangements. Whatever the schedule needs to be.

Do you work with hospice?

Yes. We coordinate alongside your hospice team to provide companion and personal care — so the hospice nurse can focus on clinical and your family can focus on time.

What does it actually cost?

It depends on schedule, level of care, and payment path. For most insurance-covered plans, families pay $0 out of pocket. For private pay we provide a clear estimate up front — no surprises.

What if my loved one is a veteran but doesn't have benefits yet?

That's one of the most common questions. We can help start the process — including Aid & Attendance, which many surviving spouses don't realize they qualify for.
