  • Protected Health Information (PHI) — if we are providing care to your loved one, we handle PHI in accordance with HIPAA and any applicable state regulations.

  • To respond to your inquiries and provide the services you've requested.
  • To coordinate care with caregivers, family members, and (when authorized) outside providers.
  • To process payments and submit claims to insurance, Medicaid waivers, or the VA where applicable.

SMS and text messaging

If you opt in to receive SMS messages from us — for example, by texting one of our staff numbers or by checking a consent box on a form — you may receive scheduling, care, employment, or appointment-related messages. Message frequency varies. Message and data rates may apply. Reply HELP for help. Reply STOP to opt out at any time.

AGAPE does not sell, rent, share, or otherwise disclose your mobile phone number, SMS opt-in data, or consent information to third parties or affiliates for marketing or promotional purposes. Mobile information collected for SMS messaging is used solely to deliver the messages you have requested.

Sharing your information

We do not sell your personal information. We may share it with:

  • Our caregivers and clinical team, as needed to provide care.
  • Insurance carriers, Medicaid waiver programs, and the VA, as needed to coordinate and bill for services.
  • Service providers who help us operate (e.g., scheduling, billing, secure communications) — under appropriate confidentiality terms.
  • Government agencies or other parties when required by law, court order, or to protect health and safety.

Your choices

You may contact us to access, correct, or delete personal information we hold about you, subject to legal and regulatory limits (for example, we are required to retain certain care records). You may also opt out of marketing emails or text messages at any time.

Security

We use administrative, technical, and physical safeguards designed to protect information from unauthorized access, disclosure, or misuse. No system is perfectly secure, but we work to keep your information safe.

Children

Our website and services are not directed to children under 13. We do not knowingly collect personal information from children under 13.
